Abstract
This dissertation describes research into the dynamics of retention and dropout in a large residential therapeutic community. The clients studied were all continuous admissions to residential treatment during the period October 1, 1984 to April 30, 1987. This averages 76 clients per month across 31 months of data collection.;Outcome studies of residential therapeutic communities have shown them to be an effective tool in the treatment and recovery of the substance abuser. The therapeutic community, however, is experiencing a serious problem. The problem is poor retention. Dropout rates during treatment are very high. These rates average between 30 and 40 percent during the first 30 days after admission. Therapeutic communities are effective for those who remain in treatment long enough for treatment influences to occur.;The focus of this research is to shed some light on and to emphasize the importance of this problem. The research hypothesis is that the variance in retention among clients in a therapeutic community can be significantly accounted for by the variance in two main constructs. These are motivation as measured by successful participation in a pre-residential admission program and a set of dynamic variables designed to measure circumstances, motivation, readiness and suitability for treatment.;It is hoped that this research will yield an increased understanding of the nature of retention. This understanding will be used to strengthen the ability to tailor, diversify and improve treatment within the thereapeutic community modality. The goal is to increase retention, thereby improving overall outcome as well as cost-effectiveness for therapeutic communities.
